Obtaining A Signed Requisition Form For Ultrasound
Your physician will work with you and investigate your avenues of interest to produce the necessary documents for ultrasound. Once you have determined how you will proceed, your doctor will provide you with a signed requisition form with the appropriate information included and indicated.
